Russia Flora: Stem to 40 cm, slender, glabrous, with 3-6 alternate leaves and basal sheaths. Leaves to 8.5 × 2.5 cm, shortly acuminate, clasping base, well-veined. Flowers to 10, white. Inflorescence to 8 cm. Bracts narrow-deltoid to broad-lanceolate, to 3 mm, 1-2 lower to 4 cm. Sepals to 9 mm, narrowed at apex, blunt; petals broad-lanceolate, blunt. Lip to 6 mm; hypochile with 2 acute basal lobes; epichile transverse-elliptic, shortly acuminate. Column to 6 mm. Capsule to 25 mm, narrow-cylindric, glabrous.
Distribution: South Kuriles (Iturup). Open deciduous forests. Flowering June-August. General: Japan-China. Described from Japan.
Notes: Northern range limit, rare, requires protection. Often overlooked due to vegetative similarity with Epipactis papillosa.
